Iruka wasn't till episode one when he realized they were the same.
The Third no doubt he was helping Naruto, because that was probably the 4ths dying wish.

But it's mentioned he never knew his parents, so he would have had to have been only an infant when they died/disapeared/whatever. Who raised him?

Well... Konoha must have some kind of social care system. Since Naruto doesn't live in an childrens home someone must have paid for the apartment. My guess is that a part of his parents money was was used to pay the rent for the apartment when Naruto turned old enough to be living on his own.
Does the manga or the anime ever tell what happend to Kushina? Did she die from giving birth to Naruto, or was she killed in a fight? Does it even say that she is dead?

we have no idea about Kushina
we can however know she is dead because well she has never been there for Naruto and when Jiraiya and Tsunade where nostalgically talking about them they were using pass tense so shes probably dead
